We need individuals who are passionate about making a difference by helping others.    The Care Management (peer) Support Specialist provides support to the Care Management Program Supervisor, supporting enrollees with complex behavioral and chronic physical health conditions, in collaboration with one or more health plans and providers in Network180’s system of care.   This position is 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. There are no weekends, on call, or holiday requirements for this position. This assignment is in person at 790 Fuller Ave Grand Rapids, Michigan 49503 and within the community.   MINIMUM QUALIFICATIONS:  High School diploma or general education degree (GED).  State of Michigan Certified Peer Support Specialist or be eligible to become certified by the State as a Peer Support Specialist, which requires an individual to have: Primary diagnosis of a mental health condition Have a strong personal knowledge of what it is like to have first-hand lived experience with a mental health condition that has caused a substantial life disruption Has been a recipient of mental health treatment and/or services for at least one year, with a substantial life disruption due to their mental health condition  Have personal experience in navigating complex mental health treatment services, and/or  State of Michigan Certified Peer Recovery Coach or be eligible to become certified by the State as a Peer Recovery Coach, which requires an individual to have: Long-term, stable recovery from alcohol and other drugs  Identifies with another individual based on their shared life experiences to promote recovery by re
