Develop and implement a plan for sharing this information with consumers and their families.&lt;/li&gt;&#13;&#10;&lt;li&gt;Be a communication conduit between individuals in the recovery community and service providers.&lt;/li&gt;&#13;&#10;&lt;li&gt;Work with the Clinician and, if applicable, the Case Manager, to collaborate on treatment planning, case consultation, and client needs.&lt;/li&gt;&#13;&#10;&lt;li&gt;Work cooperatively with service providers as they work to meet the needs of the families in their care.&lt;/li&gt;&#13;&#10;&lt;li&gt;Inform consumers and families about community resources and natural supports and how to utilize these in developing their recovery plans.&lt;/li&gt;&#13;&#10;&lt;li&gt;Responsible for timely Quality Assurance compliance as related to employee&amp;rsquo;s roles and functions.&lt;/li&gt;&#13;&#10;&lt;li&gt;For the purposes of coordination of care, follow community partner, PIHP, CMH, and agency guidelines when providing treatment with patients receiving Medication-Assisted Treatment (MAT) or other medical treatment.
