SUMMARY OF JOB: The ACT Peer Support Specialist will work as a member of the Assertive Community Treatment (ACT) Team. The Peer Support Specialist Position helps enhance community inclusion, participation, independence, and productivity for individuals receiving services at Pivotal. Those eligible for this position will have a lived experience and received treatment in the public mental health system. Peer Support Specialist will utilize their personal experience in recovery to instill hope and understanding for others. ACT Services are a team-based approach that provides intensive clinical, case management, medical, and psychosocial support to individuals with severe mental illnesses and/or substance use issues for purpose of increasing autonomy and decreasing the need for psychiatric hospitalization. ACT services are based on the principles of recovery and are individually tailored to meet the needs of the customer. Services are provided in the Customer’s residence or other community locations by all members of the multidisciplinary ACT team. QUALIFICATIONS/EDUCATION: High School diploma or GED. Personal/lived experience with a Mental Health Diagnosis, required. Personal/lived experience with substance use preferred. History or current recipient of mental health services (minimum of one year), required. Certification as a Peer Support Specialist preferred or willing to complete the certification process. This position helps the family/child understand the array of services available and directs them to the services determined to best meet their need. This position assures that families have access to accurate, understandable, and complete information in order to make choices regarding the services that will best meet the needs of their family/child. This position will be knowledgeable about and actively support culturally competent recovery based practices; person centered planning as a shared decision making process with the individual, who defines his/her life goals and is assisted in developing a unique path toward those goals; and a trauma informed culture of safety to aid consumer in the recovery process. WORKING CONDITIONS/ENVIRONMENT: Works in hospitals, office or in homes of consumers and their families. Close contact with persons who may not be psychiatrically stable and who may be hostile, aggressive and potentially violent. May work in shared office space with a fast paced and very stressful environment at times. May be required to travel between SCCMHA locations to support families throughout the intake process. QUALIFICATIONS: Education: High school diploma or GED; some post secondary education preferred. What we’re looking for: DRP is currently seeking full-time Certified Peer Recovery Mentors to participate in our RISE UP (Recovery, Integration, Support & Empowerment) program. The Certified Peer Recovery Mentors will combine their lived experience with professional Best Practices to provide support to clients in various stages of the Recovery process. Under the supervision of the Project Director, the Certified Peer Recovery Mentors will demonstrate a comprehensive, integrated, public health approach to the delivery of early intervention and treatment services while serving members in the community through support and coaching. The Peer Navigator will combine their lived experience with professional Best Practices to provide support to clients in various stages of the Recovery process. The Peer Navigator provides direct support to health system navigation, education, health advocacy, and linkage to relevant services, including social support services to those recently and/or long term diagnosed with HIV/AIDS. The Peer Navigator will work with participants minimally until the participant starts medical treatment but can continue to work with the participant as long as necessary to develop self-management tools and reach stabilization. The MIH Peer Navigator will provide peer support to pregnant and postpartum people who have been identified as having risk for substance misuse. This individual will also work with community partners to develop referral pathways to health system navigation, education, health advocacy, and linkage to relevant services, including social support services for pregnant individuals that need peer support. This role requires using a specialized set of knowledge, experience, training, and skills to encourage healthy attitudes and behaviors which prevent the use/misuse of alcohol, tobacco, and other drugs.
